RSVP of Tarrant County together with its sponsor, Senior Citizen Services, has effectively utilized a Lay Leader model which pairs volunteers and community partners to deliver two evidence-based wellness programs to older adults: A Matter of Balance Fall Prevention Program and Better Choices, Better Health Chronic Disease and Diabetes Self-Management. Together, these programs are reducing hospitalizations and increasing mobility and independence for seniors in Tarrant County, Texas.
